Employee Main Campus Vehicle Registration Form

All AUTHORIZED full-time and part-time NON-STUDENT employees of the institution are eligible for Employee Main Campus parking permits. Part-time student employees are NOT eligible for Employee Main Campus permits and must purchase a student parking permit when eligible. Housing students are NOT eligible at any time for Employee permits. Employees are not exempt from Sullivan University's rules and regulations governing parking. All employee parking permits will be issued by the Public Safety Department through the Student Services Office.

This form must be filled out in its entirety before a permit is issued.
